What do I need to know prior to my first visit and for follow up visits?:
Please have all of your paperwork completed prior to your first visit so that we are able to maximize our time with you!
Wear loose, comfortable clothing that makes it easy for you to move as well as for us to evaluate the areas of concern. (Tank tops for shoulders and shorts for low back, hips, knees and feet/ankles). Bring anything with you that you think we would need to see. If you are a golfer, bring your driver, if you are a runner, bring your shoes!
Your first visit will include a detailed evaluation of your condition and overall health history, and likely some treatment and initiation of your tailored home exercise program. Once a treatment plan has been established, follow up sessions will include assessment of your current status, may include manual techniques to improve your mobility or decrease pain, and specific exercise to help you achieve your goals. You and your Physical Therapist will continue to work throughout your sessions to make adjustments to your plan and focus on your discharge.
Your first session will be 60 minutes and your follow-up sessions will be between 30 and 60 minutes, depending upon the plan of care that you and your Physical Therapist have agreed upon.
Will my insurance cover this?
We are currently in network with BCBSIL PPO (Blue Cross Blue Shield of Illinois) and in the process of credentialing with Medicare. If you have Medicare insurance, please contact us so we can explain these details. We also offer a discounted self-pay rates and can provide you with a superbill to submit to your insurance for reimbursement. If you have another insurance carrier, please contact them to see if you are eligible for out-of-network coverage for physical therapy.
I am in network with my insurance at another clinic, why should I come to Motus?
At Motus Physical Therapy, we work with you one-on-one so that our entire focus is on you, and you are under the supervision of a licensed Physical Therapist for your entire session. We also have experienced clinicians. This will result in finding the root cause of your issue more directly, likely resulting in fewer visits and less overall cost. If you have a deductible that you have not met, you will pay full cost of sessions at any facility until you have met your deductible.
Where can I park for Motus Physical Therapy?
There are parking spaces directly in front of our building along Main St., along S. Fairview Ave., and also convenient parking from 11am on behind the building at 15 Prairie Ave. (location of Park Ridge Farmer’s Market).
What happens if I cancel my appointment?
The frequency of appointments that you and your Physical Therapist have designed for you is essential to your recovery. Your appointment time has been reserved just for you. If you do need to cancel, please be aware of our requirement for a 24 hour notice. If it is less than 24 hours, this will result in a $75 fee that is not covered by your insurance.
